For remote registration you are kindly requested to send:
- a scan of both pages of the Electronic PhD Student Identification Card stamped for the current academic year,
- a scan of the second page of Appendix C to the Regulations on the use of the Main Library and Library Network of the AGH UST - the signature and stamp of the supervisor and the planned date of graduation are required.

Appendix C 

A library account will be created remotely upon completing the form and sending scans of the required documents.
