Interlibrary Loan Request Form for libraries and other institutions (polish and foreign)

This form should be used by libraries and other institutions, Polish and foreign, for requesting publications (books, journals, articles, doctoral theses) or scans of materials held by the Main Library. Only form that is filled out correctly can be a base to fulfill your request.

Requests coming from outside of Poland will be mananged through the Interlibrary Loans bureau.
